- Deliver personalized in-home care services tailored to each client’s unique needs, including assistance with daily activities such as bathing, dressing, and mobility.
- Monitor and report on clients’ health status, noting any changes or concerns to healthcare professionals promptly.
- Support medication management by reminding clients of their medication schedules and ensuring proper adherence.
- Assist with light housekeeping tasks to maintain a clean and safe living environment.
- Provide companionship and emotional support to foster a positive and engaging atmosphere for clients.
- Follow care plans meticulously and document all services provided accurately for ongoing assessment.
- Communicate effectively with family members and healthcare teams to coordinate care and address any evolving needs.
