Duties

- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record patients’ vital signs, health status, and behavioral changes to ensure optimal care
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Administer medications accurately following care plans and maintain detailed documentation in compliance with HIPAA regulations
- Support patients with memory care needs such as Alzheimer’s and dementia by implementing specialized techniques and ensuring safety protocols are followed
- Use assistive devices like Hoyer lifts or other mobility aids to safely transfer residents and prevent injury
- Maintain a clean, safe environment through regular cleaning, laundry, and household chores to promote comfort and hygiene

Experience
- Prior experience in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques and ability to recognize signs of health deterioration or behavioral changes
- Certified in CPR and First Aid; experience with hospice care or working with individuals with disabilities is advantageous
- Familiarity with care plans, medication administration, vital signs monitoring, and HIPAA compliance standards
- Hands-on experience working with people with developmental disabilities, autism spectrum disorder, or memory impairments such as Alzheimer’s disease
- Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
- Strong communication skills for collaborating with healthcare professionals, family members, and team members
